After ccompleting your volunteer activity, you must complete the Record of Hours of Community Involvement form . You can get the form from a secretary in Student Services.

  • Fill in the form with the volunteer activity that you have completed.
  • Supervisors of each volunteer activity you completed must sign the form.
  • Hand the form in to a secretary in Student Services. She will record the information on your official school transcript.
